Looking for an intervention to help students track their individual attendance? Use this with at risk and chronically absent students.

We are 40% more likely to do something when we write it down. In order to help students build autonomy and awareness of their goals, use this form to help students self-monitor their attendance.

This form will help keep students set and maintain goals related to improving their attendance. It is best used with 5th-12th graders, but could be modified to work with K-5th grade students that are at risk or already are chronically absent.

To make this form even more effective, ensure a trusted adult checks the form regularly and provides positive feedback or meets with the student to help address attendance barriers when needed.

This process can provide an opportunity for conversation between the adult and the student which can lead to a meaningful one-on-one connection so that students feel supported.
